 We encourage you to explore our website and email us with any questions. Once you are committed to adopting a puppy from Willow Bark Farms,  please fill out this application form.  
We will contact you by phone or email within 48 hours of receiving your application.

If your application is approved, you will be invited to submit a $500 nonrefundable deposit to secure a spot on our Waitlist.
